Dear Parent/Guardian,

As a scholar at Centennial Academy, your scholar will participate in various school-wide sponsored field trips during the 2021-2022 school year. These community field trips will take place under the guidance and supervision of educators and chaperones of Centennial Academy. This year, these school-wide experiences will include:

Convocation @ Georgia Tech - August 25, 2021
Commencement @ The Tabernacle - May 22, 2022
Awards Day @ Georgia Tech - May 25, 2022 (Lower & Middle Academy) & May 26, 2022 (Upper Academy)
Field Day @ Georgia Tech - May 25, 2022 (Upper Academy) & May 26, 2022 (Lower & Middle Academy)

Please sign below acknowledging the following pertinent information:

I hereby give permission for my child to attend all school-wide sponsored field trips for the 2021-2022 school year. I hereby release, indemnify, and hold harmless Centennial Academy, Inc., its Board members, officers, agents, employees, teachers and authorized volunteers from and forever promise not to sue them on any and all claims, demands, rights, causes of action, liabilities, losses, damages, costs, and expenses (including reasonable attorney's fees), whether known or unknown, that I,  any other parent or guardian of the below-named scholar, or the scholar may have or may allege to have against Centennial Academy Indemnities or which may be brought against Centennial Academy Indemnities arising out of or in any manner relating to the scholar's participation with field trips, including but not limited to the rendering of emergency medical procedures or treatment. If any emergency medical procedures or treatment are required during the trip, I consent to the trip supervisor(s) taking, arranging for or consenting to procedures or treatment. I also acknowledge that Centennial Academy does not carry any insurance relative to the trip, including the cost of the trip or for injuries to the scholar. I represent that the scholar has insurance through their own insurance carrier.
